The diagnosis and program are discussed together. A plan is created with an estimate of number of appointments. The appointments are to meet goals that we set together throughout the program. We are registered with several insurance companies. We also off payment processing in; HSA, self payment, and membership payment. We ask that a credit card is placed on file in your encrypted and HIPAA medical records.
Payment is due 30 days after visit date. If your insurance plan is unable to supply payment, we ask that payment be provided for the appointment cost and insurance reimbursement will be returned to the client. Documentation is provided to the client to indicate this transaction.
